Nomination Form: Excellence in Nursing Mentorship

Nurses nominated for this award should possess and demonstrate excellence in the attributes of teaching, mentoring, and fostering the growth and development of nursing students or nurses at any stage in their career. All nominees must meet the following criteria: Yuma County Resident Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) Practicing RN (unencumbered active AZ Board of Nursing license) for at least 1 year Living to receive an award No self-nominations Must be over 18 to nominate a nurse Nomination essay must be a minimum of 250 words with a maximum of 500 words; if less than 250 words, the nomination essay will be disqualified. Duplicate nomination essays for a single nominee will not be accepted Nominees are limited to one submission per category for fairness. Duplicate nominations within the same category will not be counted (the Nomination Committee will reach out to the nominee if multiple nominations are submitted). There is a cap of 50 nominees for each award

Please provide a narrative giving specific examples (250 word minimum and 500 word maximum). Please give specific examples that surpass standard nursing practice on this nomination form so that the nominee can be distinguished from others and recognized for their accomplishments more readily.
