Nomination Form: Excellence in Nursing Leadership

Nurses nominated for this award should possess and demonstrate excellence in the attributes of communication, emotional intelligence, and integrity as seen in their service and performance as a nurse leader. All nominees must meet the following criteria: Yuma County Resident Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) Practicing RN (unencumbered active AZ Board of Nursing license) for at least 1 year Living to receive an award No self-nominations Must be over 18 to nominate a nurse Nomination essay must be a minimum of 250 words with a maximum of 500 words; if less than 250 words, the nomination essay will be disqualified. Duplicate nomination essays for a single nominee will not be accepted Nominees are limited to one submission per category for fairness. Duplicate nominations within the same category will not be counted (the Nomination Committee will reach out to the nominee if multiple nominations are submitted). There is a cap of 50 nominees for each award
